qi ju di huang wan 杞菊地黃丸

Uses: for shenyinxu (kidney yin deficient), dizziness and blurry vision, dislike bright light, tearing in the wind.
Modern application includes: retinitis, glaucuma, geriatric cataract, optic nerves atrophy, detachment of retina, sequela of concussion, high blood lipids, high blood pressure, low blood pressure. (bi-directional adjustment effect on blood pressure)
Prescription:
qi zi 90 g, ju hua 90 g , shu di huang 240 g, shan zhu yu 120 g, shan yao 120 g, mu dan pi 90 g, fu ling 90 g, ze xie 90 g.  Grind into powder and make into pills.
Take 6 grams each time with warm drinking water. Three times per day.
Prescription for decoction: qi ju di huang tang 杞菊地黃湯
qi zi 15 g, ju hua 12 g (bag separately, put in last), shu di huang 18 g,   shan zhu yu 12 g,  dan pi 9 g, fu ling 12 g, ze xie 9 g, shan yao 12 g。
Rinse herb with water. Add 3 bowls (about 1200 cc) of water. Simmer all herb except ju hua and simmer down to 90% of a bowl. Then add ju hua and simmer for another minute. Strain and use as tea while warm. Then add 2 1/2 bowls (about 1000 cc) of water to the herb and re-boil. Simmer down to 80% of a bowl. Strain and use as tea while warm. One package per day to be taken in two servings.
The above quantity of herb can be adjusted according to syndrome.
Adjustment:
1. Dry mouth and pain in the throat: add xuan shen 9 g, jie geng 9 g.
2. Achy loin and back: add niu xi 12 g, du zhong 15 g, sang ji sheng 12 g
3. Irritable and thirsty: add huang qi 9 g , ren shen 2 g.
4. Frequent urination: take out ze xie, add yi zhi ren 12 g,  fu pen zi 10 g.
5. Cough and rapid breathing:add wu wei zi 9 g,mai dong 10 g。 
There were news report that liu wei di huang wan cause blood vomiting. This formula contains mu dan pi which possesses a strong anticoagulation effect. So this formula may not suitable to use with anti-coagulants like Aspirin, heparin or warfarin (Athrombin-K, Carfin, Coumadin, Panwarfin, Sofarin, Warfilone).
Note: ju hua contains evaporating oil of camphor and borneol which possess the antibiotic effect. Cooking ju hua too long may lose the evaporating oil.
This formula is derived from adding gou qi zi and ju hua to liu wei di huang wan.

【杞菊地黃丸】
功用與主治﹕滋補腎陰,養肝明目。主治肝腎陰虛,頭暈目眩,兩目昏花,視物不清,眼珠澀痛,怕光羞明,迎風流淚等。 現代臨床應用:視網膜炎 ,青光眼,老年性白內障,視神經乳頭炎,視神經萎縮,視網膜脫落等,腦震蕩後遺症,高脂血症等 ,高血壓,低血壓。(對血壓有雙向調節作用。)
組成﹕熟地黃 240克,丹皮 90克,菊 花 90克,茯苓 90克,山茱萸120克,枸杞子 90克,山藥 120克, 澤瀉 90克。
用法用量﹕上為細末,煉蜜為丸,如梧桐子大。每次6克,溫開水送服,1 日3次。

湯劑【杞菊地黃湯】
處方﹕枸杞子 15克,菊 花 12克 (另包後下), 熟地黃 18克, 山茱萸 12克,丹皮 9 克, 茯苓 12 克, 澤瀉 9克,山藥15克。
第一次煎﹕水三碗。煎九分。溫服。
藥渣再煎﹕水兩碗半。煎八分。溫服。
加減﹕
1. 口乾咽痛:加 玄參 9 克,桔 梗  9 克。
2. 腰痠背痛:加  牛 膝 12 克,, 杜仲 15 克, 桑寄生12 克。
3. 虛煩口渴:加 黃耆 9 克, 人參 2 克。
4. 小便频数:去掉澤瀉,加益智仁 12 克,  覆盆子 10 克。
5. 咳嗽  氣促:加入 五味子9 克, 麥冬10 克。
 
【注意事項】服藥期間忌食酸性及生冷之物。
註一:菊花不可久煎以避免損失有效的揮發油,如冰片,樟腦等。菊花的揮發油是抑制病菌的主要成份。最好於煎好之後才加菊花。再煎一分鐘。
註二:杞菊地黃丸乃六味地 黄丸加枸杞子與菊 花而成。有新聞報導說用六味地黄丸引起吐血。本方中的丹皮 有抑制凝血作用不應與抑制凝血西 药如 Aspirin (乙醯水楊酸)  warfarin (Athrombin-K, Carfin, Coumadin, Panwarfin, Sofarin, Warfilone) (華富林),heparin (肝素)等同用。(by: Joe Hing Kwok Chu)
